Kinda, the didn't have the budget for actual blanks, so they were just buying real bullets, but removing all but the ignition gun powder. What likely happened is that a bullet was fired, didn't have enough momentum to make it out of the barrel, so they fired again and then hit the stuck bullet and it flew out. If I remember correctly.

Kinda, they used a squib load which is a round that doesn't properly fire, the bullet didn't leave the barrel, they reloaded the gun with a blank for a different scene without checking the barrel and the charge off the blank fired the bullet at a high enough velocity to kill him. But there was also an actor who got bored on set and played Russian Roulette with a blank and killed himself because even without a bullet the pressure of that shot at close distance is enough to cause skull trauma, especially if it had some wadding.
