I'm pretty sure he was referencing TvTropes, which has a set of tropes along the lines of "Did you just (x) Cthuhlu?" to refer to doing anything to provoke a higher being who can be considered dangerous and God-like.

In short, yes, it's just a metaphor. Nothing more.

Tecnically (and I know I am completely missing the point) you probably couldn't bite Cthuhlu's arm. Being made of non terrene matter aside, while Cthuhlu does vary in size considerably. The generally accepted "current" size (defined as the one he had when he tried to follow the Johannson ship as it left Ryl'eh) is about 55-60 ft tall. So trying to bite his arm would be like trying to take a bite out of the trunk of a good size tree, you'd never get past the curvature. You could bite the arm of Nyarlahotep (provided he was in one of his humanoid avatars, like the Black Pharoah, The Black Man, or the Dark Demon) or even Hastur (provided he was in the form of the King in Yellow, another guy you do not want to pull the mask off of) but not Cthuhlu.

But yeah biting Bleedhart on the arm, really bad idea. As they used to say in my old gaming group, that is SERIUOSLY Bashing the Balrog.
